How do I keep a carry-on bag from being gate-checked when it holds things I cannot be separated from?
I travel with medication and a piece of medical equipment, and it is important to me that the bag stays with me in the cabin. Not because of the rules — because if it goes into the hold and the bag goes missing or gets delayed, I have a real problem rather than an inconvenience.
On my last two flights the bag was gate-checked. Both times it was announced as mandatory for everyone at the gate, and both times I was slow to explain why I needed to keep it, which made the conversation harder than it should have been.
What actually causes gate-checking, and what is the effective way to handle it — ideally something that works before I am standing at the gate holding up a queue?

The single most effective habit: raise it at check-in, not at the gate.
At the desk there is time, there is a person whose job includes exactly this, and a note can be put on your booking. At the gate there are ninety people behind you and someone trying to close a flight on time. Same request, completely different odds.
What to say is short and specific: that you are carrying medication and medical equipment that must remain with you, and that you would like it noted that the bag is not to be gate-checked. Do not lead with the medical detail — lead with the operational request and give the reason second.
