400 cold emails, 3 replies, zero calls booked — where is this breaking Cold Outreach

Eleven days, 400 emails, one domain warmed for three weeks, SPF/DKIM/DMARC all passing. 341 delivered, 96 opened, 3 replies, all polite nos. Zero calls.

Product is a scheduling tool for independent physio clinics, $39/mo. I pull leads from a directory, check they already have some kind of online booking, and send four lines: who I am, one line on what the tool does, ask for 15 minutes.

Everything I read says 400 emails should turn into 8-12 calls. I'm getting a quarter of a percent. My first assumption was deliverability but the open number says otherwise. Is it the ask, the segment, or is cold email just dead at this price point?

    96 opens and 3 replies means they read it and did not care. That's a positioning problem, not a deliverability problem, and it's the easier of the two to fix.

    Your email tells them what your software does. They already have booking software — that's literally your qualifier. So the email lands as "here is another one of the thing you already have." Nobody books a call for that.

    Replace the product line with a symptom line. Something like: "you're on X for booking — are you still moving no-shows into the calendar by hand, or did you find a way around that?" You're not selling, you're asking about a specific annoyance you know exists in that stack. It costs them nothing to answer and it's about them, so replies go up a lot.

    Also: at $39/mo, 15 minutes of a clinic owner's time is worth more than your first three months of revenue. Don't ask for a call in email one. Ask a question. Book the call in email three.

      Second this. Reply rate went from about 1% to 9% for me purely by swapping the pitch line for a question about their current workflow. Same list, same domain, same week.

    Your segment is fine, your volume is wrong in the other direction. 400 emails to a scraped directory is 400 strangers. Ten emails to ten clinics where you know the owner's name, what software they run and one specific thing about their setup will beat it.

    I sent 34 emails total to get my first six customers. Each took about 12 minutes because I actually opened their site first. That's roughly seven hours of work for six customers. Sending 400 templated ones is also hours of work and you have zero.

      12 minutes each obviously doesn't scale, which I now understand is the entire point at this stage.

    Check what the email looks like on a phone before you change anything else. Clinic owners read mail on an iPhone between patients. If your four lines render as nine with a signature block and an image, you're dead before the copy matters.

    Also drop open tracking. It hurts deliverability slightly and it made you draw the wrong conclusion here — open rates have been noise since mail clients started prefetching images.

      This is underrated. 96 of 341 with prefetch inflating it could be 40 real humans. Which changes the diagnosis completely — 3 replies from 40 real reads is not a catastrophe.

    $39/mo cold outbound into clinics is a hard motion. Not impossible, but the economics need you to close a lot and churn almost nobody.

    Before you spend another month on the sequence, go where the annoyance already gets discussed out loud — practice management forums, the booking software's own user community, the local association's group. People post "has anyone found a way to stop X happening" and you answer with something genuinely useful. My first five came from that over about six weeks and none of them needed convincing.

    One structural thing: you're asking for 15 minutes without giving them any reason to believe those 15 minutes would be useful. Give them a reason. "I looked at your booking page and you've got three slot types configured in a way that will double-book you on Fridays" is a reason. It requires you to actually know the domain, which is the real work here.

      It isn't dead, it's that a templated pitch to a stranger always converted terribly and now it's measurable. And $39/mo through cold ads is a great way to spend $600 discovering your CAC is $310.
