How do I keep a carry-on bag from being gate-checked when it holds things I cannot be separated from?
I travel with medication and a piece of medical equipment, and it is important to me that the bag stays with me in the cabin. Not because of the rules, because if it goes into the hold and the bag goes missing or gets delayed, I have a real problem rather than an inconvenience.
On my last two flights the bag was gate-checked. Both times it was announced as mandatory for everyone at the gate, and both times I was slow to explain why I needed to keep it, which made the conversation harder than it should have been.
What actually causes gate-checking, and what is the effective way to handle it: ideally something that works before I am standing at the gate holding up a queue?

The single most effective habit: raise it at check-in, not at the gate.
At the desk there is time, there is a person whose job includes exactly this, and a note can be put on your booking. At the gate there are ninety people behind you and someone trying to close a flight on time. Same request, completely different odds.
What to say is short and specific: that you are carrying medication and medical equipment that must remain with you, and that you would like it noted that the bag is not to be gate-checked. Do not lead with the medical detail, lead with the operational request and give the reason second.
