The part of your post that matters most is buried: taking in the shoulders and armholes is what has been ruining these, and it is not a skill problem. Those two are the hardest things to alter after the fact and the easiest to get right before you cut, which is exactly backwards from how everyone approaches it.
Fit the shoulder, grade everything below it. The shoulder is the one measurement where you should pick the size and not compromise.
